Register as a New Patient
The Practice is pleased to accept patients from Trowbridge, Holt, Semington, Keevil, North Bradley, Southwick, Hilperton, Wingfield and Westwood and continues to grow successfully.
Our practice area covers the BA14 postcode.
If you have submitted your registration request & you find yourself in need of urgent medical attention before the registration process has been completed, please contact reception for advice.
Please be aware that any previous online access to your medical records, will be re-set to the date of your registration with Trowbridge Health Centre.
Temporary Patient Registrations
To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.

Need Proxy Access?
Proxy access was developed to allow someone other than the patient to access and manage parts of their GP online services account.
The proxy is given their own online access account (rather than using the patient’s login details). It is often used by the parents or recognised carers of young children, and recognised carers of adults.

Providing ID
We ask all new patients to provide identification upon registering. The following will be accepted as identification :
Examples of suitable ID:
- birth certificate
- marriage certificate
- medical card
- driving licence
- passport
- local authority rent card
- paid utility bills
- bank/building society cards/statements
- National Insurance number card
- payslip
- letter from Benefits Agency/benefit book/signing on card
- papers from the home office
- P45
